Attention Parents:

Any student in 6th through 11th grade that plans to participate in a sport for the 2022-2023 school year will need to have a physical on file with the school. Mary Rutan Hospital will offer free sports physicals on Monday, May 2, 2022. USV will be transporting students to the Bellefontaine hospital the morning of the event.  Students will need to sign up and have all forms completed as soon as possible.  The forms are attached but can also be picked up in the office.

Please make sure to return the forms as soon as possible.  This will allow us to schedule enough bus drivers for the trip.
