Required Paperwork

Please be advised that all students wishing to participate in athletics must have a current yearly physical on file with the school in order to participate in athletics.

OHSAA Physicals: Each athlete in grades 7th-12th must have a physical on file before they can start official practice.  Fall sports start as early as August 1st.  These physicals should be turned in to your coach or Athletic Director’s.  Chad Howard & Logan Rolston for all 7th-12th graders. You can also turn these into the HS offices to our attention.

Physicals must be completed on the form linked below.  The physical packet is 12 pages.  Make sure you sign the Lindsay’s Law and ODE Concussion Sign-off that are part of the physical packet. Turn in all 12 pages of the physical packet.  




Season Ticket Information:

Season tickets and Family Athletic passes can be purchased in person starting on the first day of school  during school hours in the Athletic office.  These passes can also be purchased by mailing or turning in the form linked below to the Athletic office starting August 1st.  Passes will be mailed back to you.  Prices and details are included on the document linked below.
